The revised NHG Guideline for Anxiety marks a clear change in direction. Psychological interventions are explicitly positioned as the first-line treatment, and when medication is used, SSRI’s are preferred over TCA’s.Just as importantly, the role of the pharmacist is explicitly defined in the process of tapering off antidepressants.
When discontinuing medication, the risk of recurrence of symptoms is relatively high. Withdrawal symptoms may also occur, such as FINISH symptoms, which can sometimes be difficult to distinguish from a relapse. Therefore, the guideline recommends gradual, patient-centered tapering in accordance with the multidisciplinary document Tapering SSRI’s & SNRI’s.
The pharmacist plays an important role by identifying withdrawal symptoms, advising on tapering schedules, and supporting medication switches. In addition, the pharmacist can help assess practical feasibility, such as available strengths and dosage forms, and collaborate with the general practitioner within the FTO (Pharmacotherapy Audit Meeting).
